Please note the waiting list on the NHS for an adult ADHD or Autism assessment is very long. You may wish to use the Right To Choose Scheme. There are various providers but capacity continues to change therefore you will need to choose the most suitabe service provider, obtain the appropriate referral forms and complete in as much detail as possible to help with your referral. Please then forward to us to action.

Only GPs, allied health professionals or socials workers can make a NHS referral.

To help us submit a referral we would appreciate if you can go to the referral section & complete the sections on the online form in as much detail as possible but do not submit. Please print this off and return to Holmwood Health Centre. We will then review your medical record and use details you have provided to submit a referral, if appropriate.

Once you have completed your forms please drop off to Holmwood Health Centre reception.

Alternatively you can submit an online admin query stating that you have a referral form to submit & we will send you a secure link. Please note this service is available in core hours only but limited when we reach capacity.
