Adult Autism & ADHD Assessment

Please note the waiting list on the NHS for an adult ADHD or Autism assessment is up to 3 years. You may wish to use the Right To Choose Scheme. There are various providers but capacity continues to change therefore you will need to choose the most suitabe service provider, obtain the appropriate referral forms and complete in as much detail as possible to help with your referral. Please then forward to us to action.

Please download the NHS Adult Autism Assessment referral form and complete all the relevant questions.

Please download the NHS Adult ADHD Assessment referral form and complete all the relevant questions. 

Please also complete a screening tool questionnaire and download a PDF of your results. This is needs to go along with your referral.

Once you have completed your forms please drop off to Holmwood Health Centre reception.

Alternatively you can submit an online admin query stating that you have a referral form to submit & we will send you a secure link. Please note this service is available in core hours only but limited when we reach capacity.
