Dear Patient,

patient surveyWe would like to know how we can improve our service to you and how you perceive our surgery and staff.

We have a focus group known as the PPG (Patient Participation Group) to let you  have your say. We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. Some of the surveys will be about other health services in our area.

We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.

We welcome new members from as broad a spectrum as possible to get a truly representative sample of the Practice's patient base - especially younger people and from all ethnic groups.

If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.

We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.

Many thanks for your assistance

PPG Committee

There is a representative committee currently made up of 10 members of the PPG who meet with the practice about four times a year.  These meetings form a very effective two-way communication channel:  if we have concerns or ideas that we feel should be raised with the Practice this is our opportunity; similarly the Practice use these meeting to seek our views on changes they're considering making, or just to keep us informed.

Our role is to do our best to represent all patients of the Practice and we would like to hear from you on any general matters about the way the Practice operates.  We cannot get involved in specific complaints or individual patient issues - these should be taken up with the Practice directly.

GP Appointment Survey

The North Hampshire Clinical Commissioning Group (CCG) is running a survey to identify what is important to patients in our area when accessing a GP.  The questionnaire is confidential and does not ask for any identifiable information eg names or date of birth.  The survey will run until 30th September and can be accessed here
